Gold Coast Health Plan’s (GCHP) Member Incentive program lets members earn gift cards for completing preventive health screenings. These screenings can help detect diseases early. Catching disease early can improve treatment success and health outcomes.
Point-of-Care Gift Cards
Check if your doctor’s office participates in point-of-care gift cards. Clinics that participate will give the gift card to members after they complete their screening. Members who get a gift card at their doctor’s office do not need to send a completed form to GCHP.

Flu Shots
Incentive
Members ages 6 months to 2 years can earn a $25 gift card for getting a flu shot. Members need to get the flu shot on or before their second birthday and between Sept. 1, 2024, and Dec. 31, 2025. Download the flyer for more information.
The Postpartum Visit and Diabetes HbA1c test member incentive programs ended on Dec. 31, 2023. Members can no longer receive a gift card for a postpartum visit or diabetes HbA1c test.
How to receive your gift card
- Schedule your routine care.
- Attend your appointment.
- Fill out your incentive form.
- Mail, fax or email in your form to GCHP. Your doctor may be able to do this for you.
- You should expect to receive your incentive in the mail in 6-8 weeks.
