Enhanced Care Management (ECM) is a whole-person, interdisciplinary approach to care that addresses the clinical and nonclinical needs of Members with the most complex medical and social needs. ECM provides systematic coordination of services and comprehensive care management that is community based, interdisciplinary, high touch and person centered.

If you are a correctional facility, out-of-network provider, pre-release care management provider, and/or an ECM providers who wants to engage GCHP in supporting those individuals who are returning to the county, please contact our justice liaison:

Becoming an ECM Provider
This ECM Provider Certification Application is intended to ensure the ECM provider provides satisfactory evidence of meeting the ECM requirements as outlined by Gold Coast Health Plan’s (GCHP) Model of Care to be certified as an ECM provider.
