Community Health Assessment and Community Health Implementation Plan
The Ventura County Community Health Improvement Collaborative (VCCHIC) conducts a Community Health Assessment (CHA) and develops a Community Health Implementation Plan (CHIP) every three years. These efforts help evaluate the overall health of Ventura County’s population and identify strategic priorities to guide collective action toward a healthier community.
Gold Coast Health Plan (GCHP) is a founding member of VCCHIC, alongside Ventura County Public Health, and plays an active role in both the assessment and planning processes. GCHP’s Community Advisory Committee contributes directly to the CHA by participating in primary data collection and providing ongoing feedback throughout the development of both the CHA and CHIP.

The 2022 Community Health Implementation Plan can be accessed here. The next CHIP will be published at the end of 2025, with implementation beginning in 2026.
