Population Needs Assessment

Gold Coast Health Plan (GCHP) conducts a Population Needs Assessment (PNA) to ensure that the health education, cultural and linguistic needs of members are met. The PNA evaluates the health status of members, adequacy of health education, quality improvement programs, and identifies health disparities and gaps in services.

Community Health Needs Assessment

The Ventura County Community Health Improvement Collaborative (VCCHIC) also conducts an annual Community Health Needs Assessment to assess overall population health and develop priorities that will inform planning for building a healthier community.
