Register Now: English Chronic Disease Self-Management Program Fall Series

Registration for the Chronic Disease Self-Management Program (CDSMP) is now open for telephonic workshops starting August 25 and virtual workshops starting October 6. Please contact Gold Coast Health Plan's (GCHP) Health Education Department at 1.805.437.5718 to register.

Workshops will be held once a week for six weeks. Members can choose between a virtual or telephonic workshop. The virtual workshop is 2.5 hours each week, and the telephonic workshop is 30 minutes each week.

English Telephonic Workshop

Dates: August 25 - September 29
Day of the Week: Thursday
Time: 3:30 p.m. - 4 p.m.

English Virtual Workshop
Dates: October 6 - November 10
Day of the Week: Thursday
Time: 10:30 a.m. - 12:30 p.m
