Register Now: Spanish Chronic Disease Self-Management Program Workshops Summer Series

Registration for the Chronic Disease Self-Management Program (CDSMP) is now open for telephonic workshops starting June 2 and virtual workshops starting July 14. Please contact Gold Coast Health Plan's (GCHP) Health Education Department at 1.805.437.5718 to register.

Workshops will be held once a week for six weeks. Members can choose between a virtual or telephonic workshop. The virtual workshop is 2.5 hours each week, and the telephonic workshop is 30 minutes each week.

Spanish Telephonic Workshop

Dates: June 2 - July 7
Day of the Week: Thursday
Time: 3:30 p.m. - 4 p.m.

Spanish Virtual Workshop
Dates: July 14 - August 18
Day of the Week: Thursday
Time: 10:30 a.m. - 12:30 p.m.
