Register Now: English Chronic Disease Self-Management Program Workshops Summer Series

Registration for the Chronic Disease Self-Management Program (CDSMP) is now open for telephonic workshops starting June 2 and virtual workshops starting July 12. Please contact Gold Coast Health Plan's (GCHP) Health Education Department at 1.805.437.5718 to register.

Workshops will be held once a week for six weeks. Members can choose between a virtual or telephonic workshop. The virtual workshop is 2.5 hours each week, and the telephonic workshop is 30 minutes each week.

English Telephonic Workshop

Dates: June 2 - July 7
Day of the Week: Thursday
Time: 2:15 p.m. - 2:45 p.m.

English Virtual Workshop
Dates: July 12 - August 16
Day of the Week: Tuesday
Time: 2 p.m. - 4 p.m
