Register Now: English Chronic Disease Self-Management Program Spring Series

Registration for the Chronic Disease Self-Management Program (CDSMP) is now open for telephonic workshops starting February 7 and virtual workshops starting March 23. Please contact Gold Coast Health Plan's (GCHP) Health Education Department at 1.805.437.5718 to register.

Workshops will be held once a week for six weeks. Members can choose between a virtual or telephonic workshop. The virtual workshop is 2.5 hours each week, and the telephonic workshop is 30 minutes each week.

Day of the Week: Tuesday
Time: 4 p.m. – 4:30 p.m.

English Virtual Workshop*
Dates: March 23 – April 27
Day of the Week: Thursday
Time: 10:30 a.m. - 12:30 p.m.
