Community Health Investments
Social determinants play a critical role in determining the health of individuals and communities. To encourage external community organizations, public agencies, and providers to deliver quality health-enhancing supportive services that address social determinants of health, Gold Coast Health Plan (GCHP) has established the Community Health Investments program to award monetary grants to qualified organizations.
In Fiscal Year 2016-17, GCHP awarded $1.5 million in grants to organizations throughout Ventura County for programs that focused on the following three social determinants of health:
- Access to quality healthcare.
- Access to quality and affordable food options.
- Neighborhood and built environments.
Grants by Focus Area
- Access to Healthcare: $1,115,327 (74%)
- Access to Food: $310,890 (21%)
- Built Environment: $75,000 (5%)
GCHP continues its support of organizations in Ventura County through its sponsorship program.
We are is currently accepting applications to assist mission-driven organizations like ours that are dedicated to serving Medi-Cal members. If you are a Ventura County community-based organization that we have funded in the past or a new organization that would like to partner with GCHP, we would welcome the opportunity to hear from you.
